START OF SEASON FORMS
In order to participate in any sports, the following (4) forms must be submitted to the Varsity Coach or Athletic Director by the first day of tryouts.

Athletico Physical Therapy is proud to be the official provider of Physical Therapy and Athletic Training for Shrine Catholic High School and Academy. Our athletic trainer is Nathan Forshaw, and he can be contacted at [email protected].
Please make sure you have an updated physical on file at the school before Aug. 12 to be able to practice. For it to be a valid physical it needs to be after April 15, 2024.
